This usually happens when you have selected Single List mode. Single list usually renders in one column with most styles. For multiple columns, choose All Lists or List category from the ShortCode generator.

Not directly. List items belong to Lists. A List can belong to multiple categories. You can also copy a Link to multiple Lists easily. Lists are the building blocks of this plugin not individual List items.

A Listing needs to be approved first before you can deny it. Otherwise, you can just delete that.
The Embed page is supposed to be empty. This blank page is used to generate the embedded contents on.
