The most common reason for failed import is encoding. The CSV file itself and characters in it must be in utf-8 format. Please check your CSV file for any unusual/non-utf-8 characters. If the problem persists, please email us the CSV file.

Not directly. List items belong to Lists. A List can belong to multiple categories. You can also copy a Link to multiple Lists easily. Lists are the building blocks of this plugin not individual List items.

A Listing needs to be approved first before you can deny it. Otherwise, you can just delete that.
The Embed page is supposed to be empty. This blank page is used to generate the embedded contents on.
